Evidence

Strong: Clinically the most pharmacologically active herb in the aphrodisiac chapter -- works fast and measurably. But the safe dose window is narrow and regulatory bodies have restricted or rejected it (Commission E negative monograph; prescription-only in several countries).

Traditional use and mechanism

West African (Cameroon, Nigeria, Gabon) traditional aphrodisiac. Bark bark tea used for sexual dysfunction and as a stimulant. Became pharmaceutical in the 1980s–1990s for erectile dysfunction before sildenafil. Documented in multiple Western medical pharmacopeias.

Safety

SERIOUS RISK PROFILE. Documented adverse effects: elevated blood pressure, tachycardia, palpitations, anxiety, panic attacks, hypertensive crisis in vulnerable individuals. Narrow therapeutic window. Regulatory status: Banned/restricted in Canada, Schedule 4 prescription in Australia, restricted in Germany. Available OTC in US but with unreliable dosing in supplements.
